Inspector Notes

A monitoring inspection was initiated on 10/12/2023 and concluded on 10/12/2023 from 10:20 AM to 1:20 PM. There were 85 children present, ranging in ages from infant to five, with 21 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the three staff?s records were reviewed for updates.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-770-60-C-2
Based on record review and interview, the center failed to obtain the findings of a central registry within 30 days of employment for one out of five staff records reviewed.

Evidence:

1. The record for staff 3 documents the employment date as 9/1/23. The file for staff 3 did not contain findings for a central registry.
2. The director stated the search was submitted but never received. The director stated they had not followed-up to see the status of the findings.
Plan of Correction: In the future if the findings of a central registry have not been received within 30 days the administrator will follow-up to find the status of the findings.

Based on interview and document review, the center failed to obtain an annual approval from the health department.

Evidence:

Per the director the most current health inspection the center has is dated 3/4/22.
Plan of Correction: The health department will be contacted to schedule an inspection.
