Inspector Notes
An unannounced inspection was initiated on 07/15/2024 and completed on 08/06/2024 in response to a complaint received by the licensing office on 07/09/2024 relating to allegations in supervision. The inspector reviewed one children?s record and one staff record on-site. This inspection included document review, tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please return a signed and dated copy of each page of this notice to the licensing inspector no later than 5:00pm on 08/28/2024. If you?d like to include a plan or correction and date to be corrected for each violation cited, please include that information with the signed and dated copy of the Violation Notice by the deadline. Plans of correction should include steps to correct the noncompliance with the standard and measures to prevent the noncompliance from occurring again.

2Evidence:
The licensee confirmed staff gave child 1 food they were allergic too. The staff was aware the child was allergic to the food.
Evidence:
The record for child 1 was reviewed. Per interview with staff, child 1 has a food allergy and the allergy was posted in the classroom and cafeteria. There was not an allergy care plan in the file. The director stated they did not obtain an allergy care plan for child 1.
Should a parent fail to provide documentation from their Primary Care Physician or Allergist documenting the food allergy, the child will not be allowed to attend.
