Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI. This inspection was amended on October 21, 2024 to adjust risk ratings. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 28, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-520-F
(Repeat violation) The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The list of children's allergies, sensitivities and dietary restrictions available in each classroom was not dated.
Plan of Correction: Current lists were dated for each child in all classrooms, kitchen and main office.
Standard 8VAC20-790-600-D-1
(Repeat violation) Staff shall have current certification in CPR within 90 days of employment. Staff #6 has been employed for over 3 months and is not CPR certified.

(Repeat violation) Staff shall have current certification in first aid within 90 days of employment. Staff #6 has been employed for over 3 months and is not first aid certified.

Standard 8VAC20-790-630-C-2
(Repeat violation) In areas used by preschool age or younger children, electrical outlets shall have protective covers. Two electrical outlets were not capped in the infant/toddler room within reach of children.
Plan of Correction: The entire center has been checked again for outlet covers being in place. We will do this check weekly.
Standard 8VAC20-790-640-A
(Repeat violation) Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. In the Super Stars room, sanitizer was in the middle of the child's table and bleach/water solution was sitting on top of the small refrigerator.
Plan of Correction: Teachers have been reminded and provided locked areas for hand sanitizer and bleach water bottles.

If hazardous substances are not kept in the original containers, the substitute container shall clearly indicate the contents. A spray bottle with a clear liquid was stored on the small refrigerator in the Super Stars room. Staff stated the liquid was a mixture of bleach and water. There was no label indicating the contents.
Plan of Correction: Bleach/water containers have all been properly labeled as such.
Standard 8VAC20-790-670-H
(Repeat violation) Children under 10 years of age shall always be within actual sight and sound supervision of staff. During a 10-minute period on the outdoor L-shaped playground, Child #4 was not in sight supervision of staff at least 3 different times. Staff were observed near the steps leading into the center on the back of the building and could not see the child that was near the building on the side portion of the playground.
Plan of Correction: Teachers have been notified that there must be two staff properly positioned to see all children at all times while on this playground.

Shelter-in-place emergency procedures shall be practiced twice a year. Shelter-in-place procedures were not practiced in 2023.
