During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused monitoring inspection was initiated on January 13, 2025 and completed on January 13, 2025. The on-site inspection began at 10:27 and ended at 11:20. The inspector reviewed compliance in the areas listed above. The Department issued a Notice of Intent to Revoke Exemption Granted Under Authority of Section 22.1-289.031 of the Code of Virginia on October 29, 2024 as a result of high-risk violations. There were twenty-two children present with seven staff. The inspector reviewed five staff records on-site. This inspection included document review, tour of the facility, interviews and observations. The information gathered during the inspection determined no violations with applicable code sections. No violations were issued.
