During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused monitoring inspection was initiated and completed on March 10, 2025. The on-site inspection began at 12:35 pm and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. The Department entered into a Consent Agreement with the facility on February 5, 2025. There were twenty-one children present with six staff. This inspection included document review, tour of the facility, interviews and observations. The information gathered during the inspection determined no violations with applicable code sections. No violations were issued.
