Inspector Notes

An unannounced Renewal Inspection was conducted this date. A total of 11 children (16pts) with 1 Assistant was present today. The children had just finished lunch and were transitioning to nap time. Walk thru of the program area, crib room and bathroom area was made. Artwork the children had completed in the morning was observed drying. The children were working with the letter "A" and had painted green alligators out of the shape of the letter "A". No medication needs at this time. Some topical ointments onsite. A total of 6 children's records were reviewed, all staff and 1 household record were also reviewed. This inspection occurred approximately between 12pm and 1:45pm. Evidence:
Household #1's last Central Registry search result was dated January 2017.
Plan of Correction: A Central Registry search request will be submitted for this household member. The results of the search will be forwarded to the Licensing Inspector for review within 10 working days

Based on review of staff records, one background check was not completed every 5 years.
Evidence:
Staff #2's last Central Registry search result was dated April 2017.
Plan of Correction: A Central Registry search request will be submitted for this staff. The results of the search will be forwarded to the Licensing Inspector for review within 10 working days.

Based on staff and household record review, subsequent TB screenings were not obtained.
Evidence:
Staff #1, #2, #3 and Household #1 did not obtain a subsequent TB screening every 2 years.
Plan of Correction: All TB screenings will be completed by the end of this month.

Based on observation of the family day home, one area was not maintained in an operable condition.
Evidence:
The smoke detector located in the crib room was beeping and requires a battery replacement.
Plan of Correction: A new battery will be installed by this weekend.

Based on review of child records, documentation was not found in children's records.
Evidence:
The provider failed to document the method of verification of Child #2, #3 and #4's proof of identity.
Plan of Correction: We will document the method of proof of identity, sign and date the day we reviewed the birth certificate.

Based on review of the Provider's emergency plan, an annual review/update was not completed.
Evidence:
The Emergency Plan was last reviewed in 2019.
Plan of Correction: The emergency plan will be reviewed and signed and dated.
Standard 8VAC20-800-830-C
Based on review of the provider's emergency drill log, shelter in place procedures were not practiced twice a year.
Evidence:
No documented practice of shelter in place procedures was found for 2021.
Plan of Correction: One shelter in place drill will be completed this month. A second drill will be completed in beginning of 2023.
