During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 12/10/2024 from 4:15 p.m. to 5:30 p.m. The inspector reviewed compliance in the areas listed above. There were 59 children present and 5 staff. The inspector reviewed 6 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of Child 1, Child 3, and Child 6 who have been enrolled for more than 30 days.
Plan of Correction: Parents will be reminded to provide copies of the physicals.
Standard 8VAC20-780-560-M
No child shall be allowed to drink or eat while walking around. On 12/16/2024, the young school age group was eating snack and drinking juice while putting up chairs and getting in line to go outside.
Plan of Correction: Staff will be reminded to have children sit while eating and drinking.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child 6, enrolled for four months, was missing the following required components: name, home address, and home phone number of each parent who has custody, work phone number and place of employment of each parent who has custody, name and phone number of child's physician, name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached, previous child day care, name of any additional programs or schools that the child is concurrently attending and the grade or class level.
Plan of Correction: Staff will request the missing information from the parent.
