Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/19/2021 and concluded on 5/21/2021. The director was contacted by telephone to initiate the inspection. There were 113 children present and 19 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the Violation Notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 4pm on Tuesday, June 1, 2021. Please email me at morgan.bryson1@dss.virginia.gov with any questions.
Violations
1Evidence: Staff #2 (Date of Hire: 2/3/2020) indicated on a signed Sworn Disclosure Statement on 2/3/2020 that they have resided in California within the past five years. The center requested the California criminal history record check on 5/21/2021 and results have not yet been obtained.
Quality Business Partner will submit the authorization application for Kindercare locations in VA in attempt to receive an ORI so that California will provide this documentation to center. QBP will notify the Fairfax liason of the results when complete.
