Inspector Notes

An unannounced joint inspection was conducted with the director and a representative from the local agency to investigate a self-reported incident regarding forbidden action that occurred on 6/11/2024. The joint investigation was initiated with the local agency on June 18, 2024. Facility document reviews and onsite interviews occurred on July 1, 2024. Additional interviews were conducted onsite and via telephone on July 9, 2024. Based on the preponderance of evidence, it was determined that the allegation regarding a forbidden action was valid.

Based on staff interviews, the facility failed to ensure that the following acts or threats of are forbidden physical punishment, striking a child, roughly handling or shaking a child, restricting movement through biding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment. Evidence: Evidence gathered through staff interviews reports that Staff # 1 was observed striking 4 year old, child A, during an incident on June 11, 2024.
Plan of Correction: Partnering with CPS and Licensing to report/monitor situation. Teacher was removed for the remainder of the day of the incident. Staff was retrained and reviewed Hot Button Behaviors, Look at Self, Root Causes of Physical Outbursts, Reframing How We Think About Behaviors. An alternate classroom placement for teacher involved in the incident.
