Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/17/2024 and completed on 9/17/2024. The on-site inspection began at 7:55am and ended at 9:00am. The inspector reviewed compliance in the areas listed above. There were five children present and two staff. The inspector reviewed four children?s records and two staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The family day home is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #2, employed for more than eight months, did not have documentation of requesting a central registry check or criminal history check from a different state.
Plan of Correction: The caregiver will obtain central registry checks and criminal history checks for staff #2.
Standard 8VAC20-800-140-B-3
Providers licensed after and substitute providers employed after June 30, 2010,
shall have current certification in cardiopulmonary resuscitation (CPR), as appropriate
to the age of the children in care.

The provider's certification in cardiopulmonary resuscitation had expired.
Plan of Correction: The caregiver will obtain certification in CPR.

Providers licensed after and substitute providers employed after June 30, 2010,
shall have current certification in first aid.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a
current Report of Tuberculosis Screening form, in accordance with the
requirements in 8VAC20-800-170, every two years from the date of the first
screening or more frequently as recommended by a physician or the local health
department.

The provider's most recent tuberculosis screening was more than two years ago.
