Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 11/15/2021 and concluded on 12/01/2021. The director was contacted by telephone to initiate the inspection. There were 5 children present at a total of 11 points and the licensee. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff and one household records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program..

Standard 22VAC40-111-90-A
Based on review of two children's record, the provider failed to ensure that all immunizations were obtained by the first day of attendance.

Evidence:
1. Child # 2 start date was 8/16/2021 and the immunization record was dated 11/19/2021.
Plan of Correction: The provider stated that this child is only as needed and does not attend on a regular basis. The provider also stated that there was an immunization record from before the start date and the provider believes it was discarded when the new record was received.
