Inspector Notes
A monitoring inspection was conducted on 10/3/2023 from 10:43 am to 12:16 pm. Seven children were in care with a total of 15 points with one caregiver. The Licensing Inspector reviewed three children?s records, one staff record, and one household member record, along with other program records. The children were observed returning from a walk, playing, having lunch, and getting ready for nap.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

The TB screenings for Staff 1 and household member 1 are both dated 7/25/2021.
EVIDENCE:
The record for Child 3 has not had a review of emergency contact information since the child stared on 8/16/2021.
EVIDENCE:
Child 1 began attending on 5/5/2023 and the record does not show documentation that the written provisions of the emergency preparedness and response plan have been provided.
