Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/03/2025. The on-site inspection began at 11:50 am and ended on 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 1 caregiver; the point total was 15. The inspector reviewed 5 children?s records, and 2 caregiver/household members records on-site.

The inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children?s records shall contain the information required by the standards.

The records for Child 1, 3 and 5 were missing required information.
Plan of Correction: Will send a new form home for completion.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

The immunization record for Child 5 was dated 2 days after the child?s first date of attendance.
Plan of Correction: Moving forward, will get immunizations before attendance.
