Inspector Notes

Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on June 22, 2022 from 2:08p.m. through 3:14 p.m. There were thirty-seven children in the care of nine staff. Four children's and three staff records were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were: snack, free play in the classrooms, and teacher lead group work. Interactions between the children and staff were observed as appropriate and the children seem comfortable with their daily routine and schedule.

Based on record review and verification, the center did not obtain a copy of the results of criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff A's record review showed the Sworn Disclosure indicated that Staff A lived outside of Virginia in the past five years. Staff A listed the states New Hampshire and Massachusetts.
2. Staff A's record did not have documentation of out of state background searches for New Hampshire or Massachusetts prior to 1st date of hire. Staff A's date of hire was 04/19/2022.
3. Administration verified that there were no out of state background checks completed for Staff A.
Plan of Correction: I will be sure to look at the Sworn Statement after new staff have filled it out to see if they have lived outside of Virginia in the last five years. I will be sure to run all out of state background checks in a timely manner.
