Inspector Notes

A renewal inspection was initiated on 05/09/2023 and concluded on 05/09/2023. There were 35 children present, ranging in ages from 5 years to 11 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center failed to obtain fingerprint background check search results every five years for all employees.

Evidence:
1. The record of Staff #1 (DOH: 06/08/18) did not contain documentation of a repeat fingerprint background check. Staff #1 has been employed more than 5 years.
