Inspector Notes

A monitoring inspection was initiated on 12/11/2023 and concluded on 12/11/2023. There were 42 children present, ranging in ages from 5 years to 12 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
1. The record of Staff 1 contained documentation of a tuberculosis result dated 09/14/2020.
2. The record of Staff 2 contained documentation of a tuberculosis result dated 10/09/2020.
3. The administrator confirmed that both staff members had not updated their tuberculosis results.
