Violations
2Based on review of three caregiver records, the provider did not obtain documentation of the results of a central registry search within 30 days of employment for all staff members.
Evidence: The record for caregiver #2, with a start date of 1/3/23, did not contain documentation of the results of a central registry search at the time of inspection.
Evidence: A large metal accordion expansion gate, approximately 6 feet in width, was located at the top of the concrete stairs that lead down from the playground area into the residence. At the time of inspection a child placed their head in one of the diamond shaped openings.
