During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 1/23/25. The on-site inspection began at 9:10 a.m. and ended at 10:30 a.m. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 17. The inspector reviewed five children?s records and two caregiver, and two household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/10/25. A POC submitted after this date will not appear on the public website.

Providers must obtain a completed national criminal background check prior to the employee?s first day of employment.

Caregiver #2, that began employment on the day of inspection, did not have a completed national criminal background check.
Plan of Correction: Caregiver #2 will no longer be employed until we receive approved background check prior to starting employment

Providers must maintain a record for each caregiver with all required documents.

The record for Caregiver #2, that began employment on the day of inspection, did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction: Caregiver #2 is no longer employed. I will make sure prior to employment that all required records are obtained.

Repeat Violation

Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.

In the bathroom, there was a nine-inch section of the door frame that had broken off, exposing a hole in the wall and splintering wood that was within the children?s reach.
Plan of Correction: Door frame has been replaced. Family Home Daycare will ensure it is being maintained in a safe condition.

Providers must keep a written record of children in attendance each day.

There were six children in attendance, the written record reflected five.
Plan of Correction: We will moving forward have written record of children in attendance sheet next to entrance door to have more control and ensure child added as they enter.

Repeat Violation

Highchairs shall be used according to the manufacturer?s instructions and when occupied by a child, a safety strap shall be used and securely fastened.

Child #5, two-years-old, was observed sitting in a highchair without the safety strap fastened.
Plan of Correction: After inspection high chair safety strap no longer works properly, the high chair has been removed.
