During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and concluded on June 10, 2025. The on-site inspection began at 3:32 p.m. and ended at 4:50 p.m. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 caregivers; the point total was 17. The inspector reviewed 4 children?s records, and 4 caregivers records on-site. This inspection included document review, tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 26, 2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Caregiver #2, employed for seven months, did not have a completed national criminal background check.
Plan of Correction: I will make sure to complete prior to employers first day of employment.
Standard 22.1-289.035-B-3
The provider must request a search of the central registry prior to the employee's first day of employment. Caregiver #3, employed for four months, did not have a central registry search requested. Caregiver #2 did not have a search requested until a month after hire.

Standard 8VAC20-800-120-B
Providers must maintain a complete record for each caregiver. The records of caregiver #2, employed for seven months, and caregiver #3, employed for four months, did not contain a second reference for employment. There was no name, address, and phone number of a person to contact in an emergency for caregiver #3.

Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. The tb screening for caregiver #2 was two months late and the tb screening for caregiver #3 was a week late.

Standard 8VAC20-800-240-A
The provider shall ensure that areas and equipment of the home, inside and outside, are maintained in a clean, safe and operable condition. There was a hole in the wall accessible to children's fingers.
Plan of Correction: The hole is filled and close.
