Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on March 22, 2021 and concluded on March 23, 2021. The director was contacted by telephone to initiate the inspection. There were nine children present and three staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and four staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-185-60-A
Based on review of two children's records, the facility did not obtain all required information for one child's record.

Evidence:

The record of child #2, enrolled 11/16/2020, did not contain a second emergency contact listing the name, address and phone number.
Plan of Correction: Per the director: I will contact the parent and obtain the missing information.
Standard 22VAC40-191-60-B
Based on review of four staff records, the facility did not obtain a completed sworn statement prior to the 1st day of employment.

Evidence:

Staff #3, hired 12/03/2021, had documentation of a sworn statement dated 03/19/2021 exceeding the requirement to obtain prior to the first day of employment.
Plan of Correction: Per the director: I will ensure staff complete all background checks and required documentation in the required time frames.
Standard 63.2(17)-1720.1-B-4
Based on review of staff records and interview, the facility did not obtain a copy of the results of a criminal history record information check, and a sex offender registry check from any state in which the individual has resided in the preceding five years prior to hire.

Evidence:

1. Staff #1, hired 01/18/2021 and identified as having lived out side the state of Virginia in the past five years, did not have documentation of results of an out of state sex offender registry prior to hire.
2. Staff #1, hired 01/18/2021 and identified as having lived out side the state of Virginia in the past five years, did not have documentation of results of an out of state criminal history name search prior to hire.
Plan of Correction: Per the director: A sex offender registry check was completed on 03/26/2021. (documentation was provided to the licensing inspector. A request for the criminal registry name search will be completed within 10 days.
