Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on March 22, 2021 and concluded on March 23, 2021. The director was contacted by telephone to initiate the inspection. There were nine children present and three staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and four staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations
3Evidence:
The record of child #2, enrolled 11/16/2020, did not contain a second emergency contact listing the name, address and phone number.
Evidence:
Staff #3, hired 12/03/2021, had documentation of a sworn statement dated 03/19/2021 exceeding the requirement to obtain prior to the first day of employment.
Evidence:
1. Staff #1, hired 01/18/2021 and identified as having lived out side the state of Virginia in the past five years, did not have documentation of results of an out of state sex offender registry prior to hire.
2. Staff #1, hired 01/18/2021 and identified as having lived out side the state of Virginia in the past five years, did not have documentation of results of an out of state criminal history name search prior to hire.
