Inspector Notes
A self reported other monitoring inspection was initiated on 10/30/23 and concluded on 10/30/23. The inspector reviewed compliance in the area of staffing and supervision. The inspection started at 3:00pm and concluded at 3:30pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on discussions the center failed to ensure the care, protection, and guidance of a child as required.
Evidence:
On 10/30/23 at approximately 11:47am a child was left outside on the playground by themself when the rest of the class and staff went inside. When the head count was completed inside, the staff realized a child was unaccounted for. Staff went back outside where the child was sitting in the tree playground structure. The approximate time the child was unattended was 7 minutes.
Staff did not ensure the care, protection, and guidance of this child as required.

Plan of Correction: Staff provided their explanations of the event and licensing was contacted as required, Staff have been provided a refresher on playground safety /supervision and the site will also be implementing a form for posting the number of children on the playground at all times.
