Inspector Notes
An unannounced monitoring inspection was initiated on 1/27/2025 and completed on 1/29/2025 in response to a self-report received by the licensing office on 1/24/2025 relating to Programs. The inspector reviewed one child's records and two staff records on-site and electronically on 1/27/2025. This inspection included document review, tour of the facility, interviews and observation.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The roughly handling of a child is a forbidden action.
On January 22, 2025, Staff #1 was observed grabbing Child #1 by the arm and turn to pursue another child. While pursuing the other child, Staff #1 pulled Child #1 behind them and Child #1 is observed hitting a table and a shelf.
On January 22, 2025, Staff #1 can be seen grabbing Child #1 by their left arm and while holding Child #1's arm, Staff #1 began to pursue another child. While pursuing the other child, Staff #1 is still holding Child #1's arm, dragging them into a table and a shelf.

Plan of Correction: Staff #1 was terminated and center reported incident to proper agencies.
