An unannounced monitoring inspection was initiated on 2/24/2025 and completed on 2/28/2025 in response to a self-report received by the licensing office on 2/24/2025 relating to Administration and Special Care Provisions and Emergencies. The inspector reviewed one children?s records on-site. This inspection included document review, tour of the facility and interviews.
