During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/10/2025, as a part of the licensure period, The on-site inspection began at 10:55 am and ended at 11:50 am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers. The inspector reviewed 4 children?s records and 2 caregiver/household member(s) records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.


Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Child resistant protective covers shall be installed on all unused outlets accessible to children under five years of age.

There were six uncovered outlets in the nap room and 1 outlet not covered behind crib next to gate on main level.
Plan of Correction: Corrected during inspection.

Infant high chairs shall be used according to manufacturer's instructions and when occupied by a child , a safety strap shall be used and securely fastened.

Straps of 4 highchairs and booster seats were not securely fastened while children were watching TV and eating snack.
Plan of Correction: I will remind them to use straps.

Caregivers shall wash their hands with soap and warm running water.

Caregiver did not wash hands after changing diaper.
Plan of Correction: I will remind them.
Standard 8VAC20-800-980-A
General written permission shall be obtained from the parent of each child for the provider to take the child off the premises of the family day home. The general written permission shall be on a form that lists regularly scheduled trips (e.g., library, store, playground) and the driver, if the child is to be transported.
ChildA and Child B did not have written permission obtained by parents to transport children from school to program.
Plan of Correction: I will ask parents to write note at pick up.
