During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was completed on 10/7/2025, as a part of the licensure period. The on-site inspection began at 2:10 pm and ended at 3:10 pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers. The inspector reviewed 4 children's records and 4 caregiver/household members records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Staff A's TB screening expired approximately three weeks prior to inspection.
Staff B's TB screening expired approximately five months prior to inspection.
Staff C's TB screening expired approximately three weeks prior to inspection.

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.

There is peeling paint on window sill in child play area.
Plan of Correction: We will sand.
Standard 8VAC20-800-690-D
The diaper surface shall be nonabsorbent.

The diaper changing pad had a tear.
Plan of Correction: I will get new one.
