Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/18/21 and concluded on 5/20/21. The director was contacted by telephone to initiate the inspection. There were 127 children present between the ages of three months to nine years and 24 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children?s records and five staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations
2Evidence: 1. Staff A, with a start date of 1/25/21, disclosed on their sworn statement that they had resided in New Jersey and California within the past five years. The criminal history record check from California that was submitted for the staff was dated 5/17/21 and did not yet have results on record; the results of the sex offender registry check from California were dated 5/17/21 and the results of the check from New Jersey were dated 5/18/21; the results of a search of the central registry for California were not on record.
2. Staff B, with a start date of 3/22/21, disclosed on their sworn statement that they had resided in Florida within the past five years. The results of a search of the central registry for Florida were not on record.
3. Staff C, with a start date of 5/10/21, disclosed on their sworn statement that they had resided in Alabama and Texas within the past five years. The criminal history record check for Alabama that was submitted for the staff was dated 5/18/21 and did not yet have results on record and the check for Texas was dated 5/17/21; the results of the sex offender registry check for Alabama were dated 5/17/21 and the results for Texas were dated 5/17/21.
