During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated and completed on 12/19/2024 in response to a self-report received by the licensing office on 12/18/2024 relating to Behavioral Guidance and Forbidden Actions. The inspector reviewed one children?s record and three staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 12/17/2024 at around 4:30 pm in the toddler classroom, Staff A observed child A hit a classmate in the face. Staff A then reportedly smacked child A on the left side of the face. Child A reportedly cried for about one minute after the incident.

The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.

On 12/17/2024 Staff A smacked Child A (age 21 months) on the left side of the face.
