During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the inspection. Unless otherwise noted as a violation within this inspection report the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated on 9/26/2024 and completed on 9/30/2024. The onsite inspection began at 4:00 pm and ended at 5:43 pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 2 staff. The inspector reviewed 5 children?s records on site and 2 staff records electronically on 9/30/2024.
The inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required shall be posted in a conspicuous place on licensed premises. The findings from the most recent inspection, dated 1/26/2024, were not posted in a conspicuous place on the premises.
Plan of Correction: The site director will post the results of the current inspection, upon receipt, in a conspicuous location.
Standard 22.1-289.058
Each building that houses a child day program that serves preschool-age children is required to be equipped with at least one carbon monoxide detector.
The licensed program had a preschool aged child present during the inspection and was not equipped with a carbon monoxide detector.
Plan of Correction: A portable carbon monoxide detector was ordered and should be received and in place at the facility by 10/4/2024.

Repeat Violation. Staff are to complete orientation training appropriate to the age of the children in care and including all required facility specific topics prior to working alone with children and no later than 7 days of the date of assuming job responsibilities. Staff #1?s record contained documentation of orientation training which was completed over 7 months past due. Staff #1?s record contained documentation of orientation training which was completed over 4 months past due.
Plan of Correction: The program director will ensure that all new hired staff hired will complete program specific orientation training prior to working with children and within 7 days of their employment.

Repeat Violation. A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #6, who has been enrolled for 3 weeks, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: The program director will contact child #6's parent(s) on Monday 9/30/2024 and request that the allergy plan be obtained from the child's doctor and provided to the program within 10 days.
