Inspector Notes

A renewal inspection was initiated on 11/08/2023. The provider was present with 4 children (9pts). The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medications on site. A total of 3 child records, caregiver/household member records and required posted documents were also reviewed. Children were observed arriving to the program, interacting with each other and engaging in circle time. Age appropriate materials were available for the children. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the Violation Notice issued to the program. Standard 8VAC20-800-100-A
Based on record review, a physical examination was not obtained within 30 days of attendance. Evidence: 1) In 1 out of 3 records reviewed, the physical was unavailable for review. 2) Child A had been enrolled longer than 30 days.
Plan of Correction: I will obtain the physical right away.
Standard 8VAC20-800-180-A
Based on record review, and interview with the Provider, the subsequent follow up tuberculosis screening for a caregiver was not conducted within two years from the date of the previous screening. Evidence: The screening for Caregiver A expired on 3/22/2023.
Plan of Correction: I will make the appointment and obtain the screening right away.
Standard 8VAC20-800-780-B
Based on record review, emergency contact information was not reviewed by the parent annually to ensure the information was correct. Evidence, 3 out of 3 children's records reviewed did not have documentation of the annual update.
Plan of Correction: Parents will be asked to review the emergency information, sign and date the document.
Standard 8VAC20-800-80-C
Based on record review, the Provider did not document the method of verification in children's records. Evidence: In 2 out of 3 records reviewed, the date and person viewing the proof of identification were not documented.
Plan of Correction: I will review the original documents again, date/sign for verification. In the future I will sign/date verification for Proof of Identification on new students.
