Inspector Notes
An unannounced on-site monitoring inspection was conducted on 11/22/2021. There were 4 children, ages 2 years -4 years at a total of 8 points, and1 staff members present during the inspection. The inspector observed the following: arrival, circle time, free choice activities, and breakfast. A total of 3children?s records and 1 staff records, were reviewed. The inspector arrived at 8:40AM and departed at 10:20 AM.

3Evidence:
1.Child # 2 did not have an address for one of the two required emergency contacts.
Evidence:
Observed a toilet chair in the bathroom at the end of the inspection. The toilet chair had been used and was not emptied promptly, cleaned and sanitized. The licensee stated that a child used the toilet chair as the LI was arriving.
Evidence:
1.Child # 2 started the program on 2/21/2023 and the most recent immunization record in the record was 3/6/2023.
2.Child # 3 started the program on 2/7/2023 and the most recent immunization record in the record was 2/22/2023.
