Inspector Notes
An unannounced on-site monitoring inspection was conducted on 08/09/2023. There were 5 children, ages 2 years-4 years, at a total of 8 points with the licensee during the inspection. The inspector observed the following: free choice activities, lunch and nap. A total of 4 children?s records and 1 staff record, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 10:15 AM and departed at 12:30 PM.

2Evidence:
Three children napping in the back room was sleeping on a play mat that used as the flooring for the room. The children did not have an individual mat for nap time.
Evidence:
1.The children were observed doing an activity at the tables prior to lunch. The lunch was served, and the tables were not cleaned prior to eating.
