Inspector Notes

This inspection was conducted by licensing staff using an alternate protocol, including telephone contacts, documents review, interview, and an in-person tour of the program.
A renewal inspection was initiated on 2/14/2022 and concluded on 2/15/2022. There were 89 children present, ranging in ages from 16 months to 6 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Standard 22.1-289-036-B-4
Based on a review of staff records, 1 out of 4 staff records did not have the results of a criminal history record information check and a sex offender registry check from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff #4 (doh: 7/26/2019) did not have the results for an out of state sex offender background check for State #1 in which the employee resided in the preceding 5 years.
2. Staff #4 (doh: 7/26/2019) did not have the results for an out of state sex offender background check for State #2 in which the employee resided in the preceding 5 years.
3. Staff #4 (doh: 7/26/2019) did not have the results for an out of state criminal history background check for State #1 in which the employee resided in the preceding 5 years.
Plan of Correction: We will complete all of these in the required 10 business days.
Standard 8VAC20-780-240-A
Based on a review of staff records, all staff members required did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. Staff #2 (doh: 11/8/2021) had not completed the required orientation course within 90 days of hire.
Plan of Correction: The employee was given the link and will begin the training.
