Inspector Notes
This inspection was conducted by licensing staff using an alternate protocol, including telephone contacts, documents review, interview, and an in-person tour of the program.
A renewal inspection was initiated on 2/14/2022 and concluded on 2/15/2022. There were 89 children present, ranging in ages from 16 months to 6 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

2Evidence:
1. Staff #4 (doh: 7/26/2019) did not have the results for an out of state sex offender background check for State #1 in which the employee resided in the preceding 5 years.
2. Staff #4 (doh: 7/26/2019) did not have the results for an out of state sex offender background check for State #2 in which the employee resided in the preceding 5 years.
3. Staff #4 (doh: 7/26/2019) did not have the results for an out of state criminal history background check for State #1 in which the employee resided in the preceding 5 years.
Evidence:
1. Staff #2 (doh: 11/8/2021) had not completed the required orientation course within 90 days of hire.
