Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 3:40pm-5:00pm. There were 42 children with 12 staff within the supervision guidelines. The children were observed being dismissed, having circle time and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Based on a review of staff records, 1 out of 3 staff did not submit to fingerprinting by day one of employment.
Evidence:
1. Staff #1 (doh: 4/17/2023) had fingerprints results on file that were dated 5/1/2023 at the time of the inspection.
Plan of Correction: We will make sure employees have fingerprints by Day 1 of employment moving forward.
Standard 8VAC20-780-160-C
Based on a review of staff records, 2 out of 3 staff did not have results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing.
Evidence:
1. Staff #2's (doh: 6/7/2021) most recent documentation of tuberculosis screening on file at the time of the inspection was dated 4/21/2021.
2. Staff #3's(doh: 3/13/2020) most recent documentation of tuberculosis screening on file at the time of the inspection was dated 3/12/2021.
Plan of Correction: We will have the staff member get the TB screening right away.
