Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 12:20pm-2:00pm. There was a total of 71 children with 18 staff within the supervision guidelines. The children were observed playing with toys, being held, taking a nap and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency materials were observed during this inspection. Based on a review of staff records, 1 out of 4 staff did not submit to fingerprinting by day one
of employment.
Evidence:
1. Staff #3 (doh: 8/29/2023) had fingerprints results on file that were dated 9/1/2023 at the
time of the inspection.
2. Staff #4 (doh: 9/1/2023) had fingerprints results on file that were dates 9/7/2023 at the time of the inspection.

the person's completed sworn statement or affirmation.

Based on a review of staff records, 2 out of 4 staff did not submit documentation of a negative tuberculosis screening by day one of employment and no more than 30 days prior to employment.

at the time of the inspection was dated 10/9/2023, which is after the employee began working.
