Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/24/2025. The on-site inspection began at 12:10pm and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 103 children present and 20 staff. The inspector reviewed 10 children?s records and 4 staff records on-site on 4/24/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Staff are required to submit to fingerprinting prior to employment.

Staff #4 did not have documentation of fingerprinting results on file at the time of the inspection.
Plan of Correction: We will follow-up and place the fingerprints results into the file 4- completed on /25/2025.
Standard 8VAC20-780-240-E
Within 30 days of the first day of employment, staff must complete orientation training in first
aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in
care.

Staff #3 did not have documentation on file of completing orientation training in first aid and
cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care at
the time of the inspection. Staff #3 had been employed for 7 months.
Plan of Correction: We will have the staff member take the FA/CPR training.
Standard 8VAC20-780-350-O
The ratio for balanced-mixed-age groupings of children is one staff member for every 14 children.

There was a group of 16 children in a balanced mixed-age classroom observed with Staff #1 from approximately 12:28pm-12:34pm.
Plan of Correction: Corrected during the inspection. We will make sure to maintain ratios at all times.
