Inspector Notes

An unannounced, on-site subsidy health and safety inspection was initiated on 07/24/2024 and completed on 07/24/2024. The on-site inspection began at 10:45 and ended at 1:15. The inspector reviewed compliance in the areas listed above. There were 79 children present with 18 staff. The Inspector reviewed seven child?s records and seven staff?s records.

Based on record review and interview, the center failed to obtain documentation of additional immunizations once every six months for children less than two years of age.

Evidence:

Child 6, age 13 months, has immunization record dated 08/04/2023.
Plan of Correction: The updated immunization record was obtained the day of the inspection. In the future the administrator will ensure updated immunizations will be obtained every six months as required.
