Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the setting.

A monitoring inspection was initiated on October 27, 2021 and conducted on November 4, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from 7 months to 2 years, with 2 staff supervising for a total of 22 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Standard 22VAC40-111-60-B
Based on record review of 2 children's records it was determined that one did not contain a parent's work number.
Evidence:
1. Child 1's enrollment information contained work address for one parent but it didn't contain a work number.
Plan of Correction: I will ask parent to fill it out.
Standard 22VAC40-111-960-E
Based on observation and interview. it was determined that one infant's bottles were not fully labeled with the child's full name and formula was not labeled.
Evidence:
1, Infant B has bottles that had masking tape on that only showed part of the name. The formula container also did not include a label to indicate who it was for.
Plan of Correction: I have the others labeled and dated and will work on this.
