Inspector Notes

A monitoring inspection was conducted on 11/15/23 from 10:55 am to 12:00pm. There were 8 children (29 points) in care of 2 caregivers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 children's records and 2 staff records were reviewed. The emergency drill record, first aid supplies and other daycare documents were reviewed to ensure compliance.

Standard 22.1-289.035-A
Based on records review, the family day home did not obtain results of a central registry search (CRS) every 5 years for caregivers.
Evidence: Caregiver # 1's CRS was last completed on 9/18/18. The CRS is only valid for 5 years.
Plan of Correction: The provider followed up with CRS and due to an error in the first submission, the search was sent back. However, the provider did not resubmit the corrected form. A corrected form will be submitted.
Standard 8VAC20-800-530-F
Based on observation, soft toys were found in an occupied crib.
Evidence: An occupied crib had soft toys while an awake infant was laying in the crib.
Plan of Correction: The soft toys were removed from the crib during the inspection.

REPEAT VIOLATION
Based on records review, additional immunization records for children under two years of age were not obtained , every six months.
Evidence:
1. Child # 1's (14 months old) most recent immunization records were dated 2/20/23. Additional records were not obtained.
Plan of Correction: Additional immunization records will be obtained.
