Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 05/24/2021 and concluded on 06/03/2021. The director was contacted by telephone to initiate the inspection. There were 38 children present with seven staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Based on record review, the vendor failed to ensure that documentation of the tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
1. The tuberculosis screening for staff 1 was obtained after her hire date.
a. The TB screening was obtained on 05/02/2021 which was after her hire date of 04/27/2021.
Plan of Correction: Going forward, all new staff will obtain the results of a TB screening prior to employment.
