Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety inspection was initiated on 12/16/2021 and concluded on 12/20/2021. There were 25 children present with 3 staff. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the vendor failed to ensure that documentation of the tuberculosis screening shall have been completed within the last 30 calendar days of the date of employment

Evidence:
The TB screening for staff 1 was completed on 06/17/2021 which is more than 30 days prior to her hire date of 08/10/2021.
Plan of Correction: Going forward all staff will obtain a TB screening within 30 days prior to employment.
