Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.
A code compliance inspection was initiated on 10/15/2021 and concluded on 10/19/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 58 children present, ranging in ages from 2 years to 5 years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program. Violations
2Evidence:
1) The most recent Central Registry search request results available in the record for Staff #3 were dated 10/04/2014. The director stated that a new Central Registry search has not been requested for Staff #3.
Evidence:
1) On the date of inspection, the record for Staff #4 (start date 08/31/2020) did not contain the results out-of-state Central Registry (CPS) search request and an out-of-state Sex Offender Registry search request. Staff #4 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment. 2) The center director showed documentation of a written communication dated 09/14/2020 inquiring about the CPS process. Additional follow up or the results of a request were not available for review. According to the center director, a new CPS search request form was mailed recently.
