Inspector Notes

An unannounced, on-site renewal inspection was initiated on 9/19/2024 and completed on 9/19/2024. The on-site inspection began at 10:08 am and ended at 11:40 am. The inspector reviewed compliance in the areas listed above. There were 44 children present and 9 staff. The inspector reviewed 4 children?s records and 5 staff records. This inspection included document review, a tour of the facility, interviews, and observations.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/26/2024. A POC submitted after this date will not appear on the public website.

Based on record review and interview, the center did not obtain out-of-state child abuse and neglect search requests, criminal history records checks, and sex offender registry searches prior to employment.

Evidence:
1. The records for Staff #1 and Staff #5 did not contain results of out-of-state criminal history checks.
2. The records for Staff #1, Staff #2, and Staff #5 did not contain the results of an out-of-state sex offender registry search.
3. Staff #1, Staff #2, and Staff #5 indicated living in another state other than Virginia within the last five years on their sworn disclosure statement.
Plan of Correction: We will submit out of state background checks immediately.
