Inspector Notes

A renewal inspection was initiated on 5/18/2023 at approximately 9:30am and concluded on 6/15/2023 at approximately 2:28pm. There were a total of 5 children (12 points) observed with 1 caregivers present. The children were engaged in the following: birthday celebration, circle time toy play, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 3 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on file review, and confirmed by staff #2, the Provider did not complete a search of the central registry for themselves, their assistant, and household members every five years.

Evidence: The most recent search of the central registry for the Provider, Assistant, and Household Member was completed 5/2/2018.
Plan of Correction: Provider will obtain the search results of the central registry for themselves, their assistant, and household members.

Based on children's records, and confirmed by Provider, children's records did not contain name, address, and telephone number of each custodial parent's place of employment.

Evidence: There is no work phone number for each custodial parent of Child #3.
Plan of Correction: Provider will obtain the work phone numbers for the parent of Child #3.
