Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/11/2024 and completed on 9/11/2024. The on-site inspection began at 11:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were four children present and one staff. The inspector reviewed four children?s records and two caregiver and household member records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Within seven business days of the child's first day of attendance at the family
day home, the provider shall obtain verification of the identity and age of the child.

There was no proof of identity for child #3, who had been attending for longer than seven business days.
Plan of Correction: The caregiver will obtain proof of identification and age of the child.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all
caregivers and children in care during all shifts that children are in care.

The most evacuation procedures had not been practiced since March 2024.
Plan of Correction: The caregiver will ensure that monthly evacuation procedures are practiced monthly.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations
for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

There was no documentation of additional immunizations for a child #4 who is not exempt from the immunization requirements once every six months while the child is under the age of two years.
Plan of Correction: The caregiver will obtain updated immunization records for the child.
