During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 4/8/2026. The on-site inspection began at 1:00pm and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was eight. The inspector reviewed three children?s records and two caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 7/11/2025, were not posted in a conspicuous place on the premises.
Plan of Correction: The most recent inspection notice was posted during the inspection.
Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be
maintained in a clean, safe, and operable condition. The children's swing set in the backyard had rust on the top bar.
Plan of Correction: The provider will have the rust removed.
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children. Poisonous hornet spray was on a shelf in the backyard, in an area where children could access.
Plan of Correction: The spray was moved to an area inaccessible to children during the inspection.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for children #1, and #2 did not have a parent?s signature for an annual review of the emergency contact information.
Plan of Correction: The provider will have the parent review their emergency contact information and sign acknowledgment.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all
caregivers and children in care during all shifts that children are in care. There had not been a monthly evacuation drill practiced for the previous January and February.
Plan of Correction: The provider will remember to practice emergency evacuation drills each month and will plan the drills around the weather.
