During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 2/24/25. The on-site inspection began at 10:55 a.m. and ended at 11:44 a.m. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was four. The inspector reviewed two children?s records, one caregiver record, and one household member record on-site. This inspection included document review, tour of the facility, interviews, and observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/12/25. A POC submitted after this date will not appear on the public website.
Violations
3Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.
At the bottom of the stairs of the daycare area a vent was observed, two of the slats of the vent had a gap wide enough to pose a finger entrapment hazard.
In the bathroom of the daycare area, a mildew cleaning product and a cleaning rag was observed on the bathroom sink of the daycare
Before the child?s first day of attendance, parents shall be provided required information in writing.
The parents of Child #1 and Child #2 did not have signed provisions of the emergency preparedness plan.
