Inspector Notes
An unannounced monitoring visit and facility tour was conducted. At the time of inspectors' arrival there were 17 preschool to school aged children in care with 2 staff members and 1 administrator. Children were observed interacting with staff, nap, and free play. A sample of 5 children's records and 2 staff records were reviewed. There have been no injuries and no medication is being administered. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

5Evidence:
1. The record for staff #2(hired 9/5/16) did not contain documentation of an updated Central Registry search. The documentation available was dated 7/29/16.
2. Staff confirmed updated documentation was not available during this inspection.
Evidence:
1. There was peeling paint and broken drywall in the school aged room.
2. In the girls restroom there was broken floor tile and a toilet with no lid on the tank.
3. In the boys restroom there was missing floor tiles and rust on the partition.
Evidence:
1. There was Lysol and Clorox on a high shelf in the school aged class restroom.
2. There was Lysol in an unlocked cabinet under the sink in the girls restroom.
Evidence:
1. Both staff members present during the inspection did not have current CPR certification. The CPR certification documentation available was dated 8/11/2018.
2. Staff stated that they were unable to attend the training previously scheduled for 8/28/21 and they had difficulty scheduling with someone.
