During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/12/2025, as a part of the licensure period. The on-site inspection began at 9:30a.m. and ended at 11a.m. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 7. The inspector reviewed 5 children?s records and 1 caregiver and 1 household member record on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-560-A
A caregiver shall be physically present on site and provide direct care and supervision of each child at all times.
Staff #1 left a total of 5 (ages 3- 6 years) children unsupervised for approximately 1 minute.
Plan of Correction: Grand daughter was visiting and was assisting her out of the car and will not leave children unsupervised going forward.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided in writing the following information: information to parents, medication policy, emergency procedures and liability disclosure.

The file for Child #2 did not contain information to parents, medication policy, emergency procedures and liability disclosure. The child had been in care for 2 days.
Plan of Correction: This is the child's last day of attendance. Going forward all documents will be obtained at the time of enrollment.
