Inspector Notes

An unannounced monitoring inspection was initiated on 5/6/2025 and completed on 5/14/2025 in response to a self-report received by the licensing office on 4/23/2025 relating to 22.1 Religious Exempt. This inspection included tour of the facility, interview and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/20/2025. A POC submitted after this date will not appear on the public website.

The center shall ensure all areas of the premises accessible to children are free of obvious injury hazards.

The awning beside the playground contained square washers on the bottom of its frame. The corners of these washers were sharp to the touch and resulted in injury to Child #1.
Plan of Correction: Director installed PVC and foam covers over square washers and screws on the awning.
