Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on October 28, 2022, regarding allegations of forbidden action and supervision. The inspection was initiated on December 14, 2022, and completed on January 17, 2023. On December 14, 2022, the inspector was on site from 2:15 PM to 4:25 PM. The licensing inspector completed staff interviews, staff schedules, and observations were made in the center.

The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. There are no violations resulting from this inspection.
